Logan County Health District officials reported Monday that the agency at 310 S. Main St. is operating under Incident Command status and only essential services are being offered during the COVID-19 emergency.
“The purpose is to limit the amount of traffic in our building and to protect you and our employees,” officials said.
The Vital Statistics & Environmental Services’ front desk has opened up a walk-up window from the outside of the building so individuals may receive service without coming indoors. For birth and death certificates that are emergent, patrons are asked to utilize the walk-up window.
For birth and death certificates that are not emergent, patrons should call either (937) 651-6192 or (937) 651-6202 for the record to be mailed to you. If we are unable to take your call, leave a detailed message and we will call you back as soon as possible.
Funeral homes are asked to communicate with the office via e-mail as much as possible at [email protected].
For environmental services including wells, septic systems, food service, plumbing, parks, camps and building reviews, communicate with staff via e-mail as much as possible, [email protected].

For WIC Services, no one will be permitted in the building. One healthy adult should check in with a WIC Staff Member in the parking lot to receive further instructions. Park in the designated “WIC” spots to ensure you’re seen in order of arrival.
