The second cycle 2016 grant in the health care category supports the “Whole Health-Whole Person” project and is funded through a cooperative agreement with the Administration for Community Living.
“Community Health & Wellness Partners is so excited to have been awarded funds from the Christopher Reeve Foundation for the Whole Health-Whole Person project,” Tara Bair, president/CEO of CHWPLC, said in a release. “We will now be able to retrieve a more accurate weight of our patients who are confined to wheelchairs and remove barriers for physical examinations.”
