Community Health & Wellness Partners (CHWP), an independent, nonprofit health center that provides primary care and other wellness services to the residents of Logan, Champaign and surrounding counties, has announced the addition of Jasmine Usher-Ledet, MD, to its team of primary care physicians.
Dr. Usher-Ledet, a Board-Certified Family Medicine Physician, attended the Ohio State University rural program, a family medicine residency rural training track in West Liberty in 2011. She attended the Medical University of Americas in Charlestown Nevis, where she received her Doctor of Medicine degree and has an undergraduate degree in psychology.
“We are thrilled to welcome Dr. Usher-Ledet to Community Health & Wellness Partners. She is a life-long learner and teacher and has hosted medical and nurse practitioner students in her practice, teaching them the particulars of practicing medicine in a rural setting,” said CHWP CEO Tara Bair.
“These qualities, and Dr. Usher-Ledet’s commitment to rural health, make her a true asset to our practice and our community.”
For the past 10 years, Usher-Ledet has served as a family physician at Knox Community Hospital in Centerburg. She currently serves as an Adjunct Clinical Assistant Professor in Family Medicine at Ohio University’s Heritage College of Medicine.

Originally from Canada and now a U.S. citizen, she calls Logan County “home.” Dr. Usher-Ledet and her husband enjoy spending time at home with their family of dogs.
