Theresa Ann Hoffman, 59, of Bellefontaine, passed away at 5:45 p.m. Friday, September 10, 2021, at Riverside Methodist Hospital surrounded by her family. She was born in Bellefontaine, Ohio on August 6, 1962, a daughter of the late David Wayne and Catherine Marie (Ledley) Hoffman.
