Lisa Kay Smith, 47, of West Liberty, passed away at 1:52 p.m. Thursday, Dec. 13, 2018, at Mercy Health in Urbana.
She was born in Bellefontaine, on Feb. 8, 1971, a daughter of David “Junior” Smith and the late Sally L. Chapman Smith.
Lisa is survived by her father and stepmother, David “Junior” and Diane Smith; a daughter, Shelby McCullough; a son, Dylan McCullough; a granddaughter, Aniya; a sister, Melissa Ann Williams; maternal grandmother, Ann Chapman Swanson; paternal grandmother, Arlene Smith; two nephews, Landon and Linkon Williams; two aunts, Carolyn (Bill) Roberts and Dawn Chapman; and two uncles, Gary (Jane) Smith and Duane (Jackie) Smith.
In addition to her mother, Lisa was also preceded in death by a grandson, Quinton; a sister, Stacey Lynn McCullough; maternal grandfather, Don Chapman; paternal grandfather, John Smith; and three uncles, Tim, Todd and Ted Chapman.
A homemaker, Lisa was a 1989 graduate of West Liberty-Salem High School. She was a hard worker who loved being with her family, especially her children and grandchildren.
The funeral is at 12:30 p.m. Tuesday, Dec. 18, at Eichholtz Daring & Sanford Funeral Home, 101 Zanesfield Road, West Liberty, following calling that begins at 11:30 a.m. Chaplain Ray Branstiter officiates and burial follows in Fairview Cemetery in West Liberty.
