John A. Zrelak, 55, was taken to Hardin Memorial Hospital by squad where he was pronounced dead following the 2:30 p.m. crash on State Route 67 near Township Road 119, troopers of the Findlay Post of the Ohio State Highway Patrol said.
Ricky Lee Milliron, 56, of Belle Center, also was taken to HMH by squad with serious injuries. Mr. Milliron was treated for his injuries and released.
Milliron was driving a 2005 Ford Excursion east on S.R. 67 when he went left of center and struck Zrelak's 2010 Toyota Camry head-on in the westbound lane, the patrol said.
Weather appears to have played a role in the crash, troopers noted. Both drivers were reportedly wearing seat belts.
Hardin County Sheriff's Office, plus McGuffey, Kenton and Richland Township fire departments assisted at the scene.
The crash remains under investigation and charges are pending, the patrol said.
